Checking the wiring
Check the relay coil driver circuit for an intermittent short-circuit to supply voltage by measuring the voltage between terminal #2 in the relay base and relay 2/17.
For further information about a short-circuit to supply voltage.

Hint: The voltage between terminal #2 and ground must be approximately 0 volt.
