Attachment I - Overview
CONDITION OF CONCERNOn subject vehicles, the fuel tank lock ring, which holds the fuel pump in the fuel tank, and/or the fuel pump seal, may have been installed improperly. In some cases, this may result in a fuel odor from fuel leaking from the fuel pump area, causing fuel accumulation. Fuel leakage in sufficient quantity in the presence of an ignition source could potentially result in a vehicle fire.

Mazda will notify U.S. owners by first class mail beginning February 9, 2004.
PARTS INFORMATION
* Important notice: California Department of Motor Vehicles, Vermont Department of Motor Vehicles, and the Commonwealth of Massachusetts, in conjunction with the California Air Resources Board and the Environmental Protection Agency for the State of Massachusetts and Vermont, have implemented the Registration Renewal/Recall Tie-In Program, which requires the completion of Emission Recalls prior to registration renewal. Mazda dealers will provide customers with a Vehicle Emission Recall - Proof of Correction Certificate upon completion of the recall.
PARTS ORDERING
Dealers will automatically receive a quantity of Fuel Tank Lock Rings and Fuel Tank Lock Ring Seals based on the number of 2003 MAZDA6 retail sales per dealer. Parts shipments will begin February 6, 2004, and will arrive on dealers' next stock order.
To order additional parts (Fuel Tank Lock Rings, Fuel Tank Lock Ring Seals, Fuel Line Retainer (Green), and Fuel Line Cage (White), fax an order form (found in this package and on ESI under RECALLS/SSPs) to Mazda Corporate Dealer Assistance Group. Mazda will ship parts as CSO orders, and freight will be reimbursed as a sublet on warranty claims. The maximum order quantity per order form for Lock Rings and Seals is 5. The maximum order quantity per order form for the Retainer and Cage is 2.
To order a Fuel Tank, you must FIRST call the Technical Assistance hotline for authorization. Then write the authorization number on the order form and fax it to the Corporate Dealer Assistance Group. The order form must include an authorization code for the fuel tank order to be processed. The maximum order quantity per order form for the Fuel Tank is 1. Fuel tanks will also be shipped as CSO orders, and freight will be reimbursed as a sublet on warranty claims.
Use MStore to order additional Campaign labels (1 =sheet of 18 labels) and Vehicle Emission Recall - Proof of Correction certificates (1=50 certificates).
WARRANTY CLAIM PROCESSING INFORMATION

Parts ordered from Corporate Dealer Assistance Group will be shipped as a CSO order. Dealer may claim reimbursement by adding a sublet to the recall claim using the format shown.
Submit CSO Freight with the recall repair claim. Do not submit separate claims for sublet.
RENTAL CAR
If the customer was placed in a rental car while the campaign was being completed, add a sublet to the claim using the information shown.
Rental expenses incurred will require DSA or DCSM authorization if the rental exceeds two days or $26.00 per day, as outlined in the Rental Car Reimbursement Program.
